By having one-stop access to your primary care doctor, mental health services, case managers and others, you can get many of your needs met and your questions answered all at once.

We take a team approach to providing care and you are the coach of this team.

We will meet to discuss your current needs and develop a complete plan of care.

We think this complete team approach is the best way to meet your needs, particularly if you have problems with your condition.

We are here for you. We want to help you live well. Our services are completely confidential. Cost for services is based on a sliding scale according to your income.

Case Management
We offer case management services for persons with HIV or AIDS.

Our case management team will:

• Learn about your needs;
• Help you find and use needed services in the community;
• Help you stick with your medication program; and
• Educate you about HIV so you know all of your options.

All case management services are free and completely confidential.
