The Horizon Program Can Help You

The Horizon Program provides a complete range of quality medical and social services to all identified HIV-positive persons in central and mid-coast Maine.

All of these services are for people infected and affected by HIV/AIDS.

Horizon Clinic*

Horizon Clinic offers an easy way to meet with your providers in one place. This saves you time, energy and expense.

By having one-stop access to your primary care doctor, mental health services, case managers and others, you can get many of your needs met and your questions answered all at once.

We take a team approach to providing care and you are the coach of this team.

We will meet to discuss your current needs and develop a complete plan of care.

We think this complete team approach is the best way to meet your needs, particularly if you have problems with your condition.

We are here for you. We want to help you live well. Our services are completely confidential. Cost for services is based on a sliding scale according to your income.

Case Management
We offer case management services for persons with HIV or AIDS.

Our case management team will:

  • Learn about your needs;
  • Help you find and use needed services in the community;
  • Help you stick with your medication program; and
  • Educate you about HIV so you know all of your options.

All case management services are free and completely confidential.

Ryan White Part C Program*

The goal of the Ryan White Part C Program is to improve basic health care for people living with HIV in a seven-county region in central and mid-coast Maine. The program is funded through a federal grant.

Please call us if you have been diagnosed with HIV and/or receive HIV-related services in Kennebec, Somerset, Franklin, Sagadahoc,Lincoln, Waldo or Knox counties.

This program can pay for all or part of any basic health care service that is:

  • Deemed needed by your doctor; and
  • Not covered by your health insurance or another program.

The scope of services is broad because people's needs vary. These services, that we provide or assist with, include, but are not limited to:

  • Counseling
  • Dental care
  • HIV/AIDS-related eye care
  • Insurance premiums and co-pays
  • Medication
  • Nutrition assessment
  • Primary medical care
  • Transportation

To receive this care, you must meet federal poverty guidelines.

* These programs are funded in part by the Ryan White Part C Program.
